►   Patient Eligibility Guidelines

Over 25% of adult Ashe County residents and workers have no form of medical insurance. To be eligible for ACFMC services, the following three criteria must be met: 

* Ashe County adult resident or worker, ages 19 to 64. 

* Without employer or private health insurance, VA, Medicaid, Medicare or other known health benefits. 

* A limited total family income.

* Eligibility intake interviews are conducted the first time a new patient is seen at the clinic. 

* An appointment is required to be seen at the clinic, and for the eligibility interview.
